Class Registration

By signing this waiver, I, the legal guardian of the student(s) I have registered for {select-1} Class, agree to the following conditions:

I do hereby release Hobnob Theatre Company and all instructors from any and all claims for injuries which the student may sustain while participating in any activities with the Studio Hobnob Classes.

I authorize Hobnob Theatre Company to contact the persons named as emergency contact ({name-3}) to authorize any necessary medical attention for the student. I assume full financial responsibility for emergency medical care for the participant while he/she is enrolled in Studio Hobnob Classes.
